Today's Field Brief shares updates and reminders for service providers reporting data for the State Opioid and Stimulant Response grant (SOS). The grant is funded by the Ohio Department of Behavioral Health (DBH) through the Substance Abuse and Mental Health Services Administration (SAMHSA) and evaluated by the Ohio Colleges of Medicine Government Resource Center (GRC).

Data is collected with SAMHSA’s Unified Performance Reporting Tools (SUPRT) and entered into the DBH iPortal application. If providers still need to submit a request to transfer GPRA to SUPRT, email SOS4@dbh.ohio.gov with the subject “SOS GPRA to SUPRT Data Migration”. In addition, quarterly reporting tools are used to collect required federal data that are submitted through a REDCap link emailed to grant awardees.

Submit Quarterly Reports

Quarter 2 reporting will close April 10th (Friday). All organizations directly funded by DBH are required to submit quarterly data. Please see the list of SOS 4.0 Year 2 awardee reporting requirements (last updated March 19, 2026). If you do not see your organization’s name on the list, please email sos4eval@dbh.ohio.gov and CC SOSEval@osumc.edu to confirm your reporting responsibilities. Q2 reporting includes the SOR/TOR Program Instrument, SOR/TOR Sub-Recipient Report, and Mid-Year PPR. If your organization was awarded supplemental funding, you must submit a separate SOR/TOR and Mid-Year PPR. The SOR/TOR Sub-Recipient Report reports on the previous fiscal year (9/30/2024-09/29/2025) and only applies to SOS 3.0 NCE and SOS 4.0 Year 1 grant awardees. GRC will host another live Question & Answer Session for Quarterly Reporting on April 7th. GRC joined DBH during the DBH SOS 4.0 March Quarterly Monitoring Meeting to share program reminders and updates with SOS 4.0 grantees. The topics covered by GRC include:

  • Quarterly reporting updates and reminders
  • Technical assistance and who to reach out to for assistance (GRC vs. DBH)
  • Promotion of the new supervisor training
  • A sneak peek of some important slides from the supervisor training including direct awardee responsibilities and important reminders/tips
  • Overview of the gift card order form
  • How to get updates from GRC by signing up for field briefs and checking our website

Sign Up for Training


Register for the SOS Awardee & Supervisory Training

GRC is offering additional sessions of the SOS Awardee and Supervisor Training on April 14th and April 24th. This one-hour training is designed for SOS-funded Alcohol, Drug, and Mental Health Services boards, grant managers, and supervisors who oversee SOS data collection, including SUPRT and quarterly reporting. Participants will review SOS data collection and reporting requirements and clarify key evaluation roles and responsibilities. Register for the Required Two-part Training

GRC continues to offer live sessions of the required two-part SUPRT training series. All data collectors, data entry staff, and managers overseeing data collection must complete both courses:

  • SUPRT Data Collection: Learning how to collect the SUPRT (90-minute presentation with optional 30-minute live Q&A)
  • SUPRT Data Entry: How to enter the SUPRT into iPortal (one-hour presentation)

  1. Add the SOSEval@osumc.edu email address to your contacts.
